New Clients & Medication Refills
For our new clients, we ask that you fill out our online form which can be found on our New Client Form page. If you prefer to print it out, you have the option to download the PDF version and can bring it with you at the time of your appointment.
To request a medication refill, please go to our Medication Refill Request Form and fill in with your provided information. If you have any questions or run into technical issues along the way, please do not hesitate to call our staff for help. We will be happy to assist you.
